Documents required

The following documents must be submitted in order to undertake staff mobility for teaching or training, funded by the Erasmus+ programme:

Before mobility:

1. HE Mobility agreement for teaching (KA131-171)_OUT or HE Mobility agreement for training (KA131-171)_OUT
2. Mobility Declaration
3. Report before foreign country travel - to be submitted within 20 days before the date of the secondment.
4. A copy of the European Health Card and the travel assistance insurance abroad.
5. A bank statement with details of personal bank account (holder, bank, branch, IBAN, BIC)

On the basis of all documents submitted to the CIRM, a contract between the UACEG and the lecturer/employee for the financing of the mobility is drawn up. A Foreign Country Travel Order is also prepared.

After the mobility:

1. Certificate of Attendance issued by the host university with the start and end date of the mobility.
2. Documents certifying the trip (tickets, boarding passes, stamps in overseas passport, hotel invoice)
3. Submission of an on-line report (on-line EU survey) - to be completed by the teacher or staff member after receiving the request by email.

4. "My Erasmus" report - free form text within A4, plus two photos from the mobility related to the activity carried out - teaching or learning.
